a. Sponsor names: St. Louis Blues 14 Fund, Give Kids A Smile, Inc., and Delta Dental Health Theatre, Inc.

b. Price of raffle ticket: $2 (one entry) or $5 (three entries).

c. Method by which the winners will be determined and the raffle will be conducted: The drawing will be held on 3/18/2010. Winners need not be present to win. Raffle tickets can be purchased at the 2/6, 2/9, 2/12, 2/13 and 3/16 St. Louis Blues home games or online at www.smilesleague.com. Each purchased raffle ticket will contain a code. Purchasers must register their ticket code at www.smilesleague.com in order to be eligible for the drawing. Winners will be determined by a random drawing of all eligible codes and entries received by an independent judging agency whose decisions are final in all matters relating to the drawing. The winners will be notified via contact information supplied during the online purchase and registration processes. Winners will have three (3) days from notification of winning to confirm his/her eligibility and his/her acceptance of the prize won. If a selected winner is unable to be contacted after a period of three (3) days from the raffle drawing date, or if the selected winner is not eligible or declines acceptance of the prize, an alternate winner will be chosen from the remaining entries. Winners must be 18 years or older, must be a Missouri or Illinois resident, must provide a valid address and phone number for prize delivery and notification, and must sign a release, if requested. Employees and organizations associated with the raffle campaign supporting companies (e.g. Delta Dental) or the sponsoring charities or their immediate family members may not purchase raffle tickets and are not eligible to win. The sponsoring organizations, and not the prize winner, will be responsible for all applicable federal and state taxes. Prize is a jersey autographed by T.J. Oshie, a gift bag containing St. Louis Blues gifts, four (4) tickets to the 3/28/2010 St. Louis Blues game and an invitation to the morning practice on 3/28/2010. Estimated retail value: $500. No cash redemption. No prize substitution. No transfer of prize.

d. Contingency plan for inclement weather or other extenuating circumstances if the raffle or alternative raffle cannot be conducted as planned on 3/18/2010: The raffle will be completed on the next available business day by 12 p.m.

e. Redemption claim period: If the raffle prize is refused, the winner is not eligible or cannot be contacted within three (3) days of the drawing, or arrangements cannot be made with the winner within three (3) days of the drawing, an alternate winner will be drawn from entries.

f. Refund Policy: No ticket refunds will be given.
